Shin Splints or a Stress Fracture? How to Tell the Difference

Pain along your shin after running or dancing? Don't ignore it.
August is a busy month for many runners preparing for autumn races, while Irish dancers are often increasing training ahead of competitions and the new season. One of the most common complaints we see at BodyRight Physiotherapy in Drogheda is pain in the front or inside of the lower leg.
Many people immediately call it "shin splints," but sometimes the problem is more serious, such as a stress fracture. Knowing the difference is important because the treatment, recovery time, and return to sport are very different.
What are Shin Splints?
The term shin splints usually refers to Medial Tibial Stress Syndrome (MTSS). This is an overuse injury where repeated loading causes irritation of the muscles, fascia, and outer layer of the tibia (shin bone).
It commonly develops gradually when training loads increase faster than the body can adapt.
Common causes include:
Increasing running distance or speed too quickly
Returning to exercise after a break
Hard running surfaces
Poor footwear
Reduced calf or ankle mobility
Weakness around the hips or core
Biomechanical factors such as overpronation
Irish dancers are particularly prone to shin splints because of the repetitive jumping, landing, and high-impact footwork involved in training.
Symptoms of Shin Splints
Typical symptoms include:
A dull ache along the inside border of the shin bone
Pain that eases as you warm up but returns afterwards
Tenderness over a longer section of the shin (usually more than 5 cm)
Mild swelling may occasionally be present
Both legs can sometimes be affected
The pain usually develops gradually over several weeks rather than appearing suddenly.
What is a Stress Fracture?
A stress fracture is much more serious.
Instead of irritation around the bone, repeated loading has caused tiny cracks within the bone itself. This happens when the bone is being stressed faster than it can repair itself.
Stress fractures most commonly occur in:
Distance runners
Irish dancers
GAA players
Military recruits
Athletes who suddenly increase training
They are also more common if there are issues such as low energy availability, vitamin D deficiency, reduced bone density, or previous stress fractures.
Symptoms of a Stress Fracture
Warning signs include:
A very specific point of pain you can often cover with one finger
Pain that becomes progressively worse during activity
Pain that may continue after exercise
Pain when walking
Pain during hopping on the affected leg
Night pain in more severe cases
Swelling over one small area
Unlike shin splints, the pain generally does not improve once you warm up.

One useful clinical test is the single-leg hop test. Someone with shin splints may find hopping uncomfortable but manageable. Someone with a stress fracture often cannot hop because of sharp, localised pain. However, this is only one part of an assessment and should never be used as the sole diagnostic test.
Why Early Assessment Matters
Many people try to "run through" shin pain.
Unfortunately, continuing to train on a developing stress injury can allow a simple overload problem to progress into a true stress fracture, leading to several weeks or even months away from sport.
Early physiotherapy assessment can often identify:
Whether the injury is likely muscular, tendon-related, periosteal, or bony
Training errors
Running mechanics
Footwear issues
Muscle weakness
Mobility restrictions
Factors that may have contributed to the injury
If a stress fracture is suspected, we will advise referral for appropriate imaging and liaise with your GP or sports medicine specialist if required.
Treatment
Treatment depends entirely on the diagnosis.
Shin Splints
Treatment may include:
Temporary modification of training
Progressive calf strengthening
Hip and core strengthening
Improving ankle mobility
Manual therapy where appropriate
Running gait assessment
Footwear advice
A gradual return-to-running programme
Stress Fracture
Treatment focuses on allowing the bone to heal safely and may involve:
Activity modification or temporary unloading
Appropriate medical referral
Gradual reintroduction of weight-bearing exercise
Progressive strengthening
Return-to-running rehabilitation
Addressing nutrition and training load if appropriate
Returning too early greatly increases the risk of the fracture progressing.
Prevention
The best prevention is sensible training progression.
Remember these simple principles:
Increase weekly training gradually.
Replace worn-out running shoes.
Build calf strength and hip strength.
Include recovery days.
Avoid sudden spikes in training.
Listen to persistent pain rather than pushing through it.
For Irish dancers, incorporating strength and conditioning alongside dance training can significantly reduce the risk of overuse injuries.
When Should You See a Physiotherapist?
If your shin pain:
Lasts more than a few days
Keeps returning
Is becoming more severe
Stops you running or dancing
Is painful when walking or hopping
it's worth getting assessed before the injury becomes more serious.
