The Benefits of Belly Breathing: Why We Prescribe It for So Many Issues


Belly breathing, diaphragmatic breathing, 360 breathing—there are many names for breathing deeply into the lower lungs and allowing the diaphragm to do the work it was designed to do.

At BodyRight Clinic, this is one of the most commonly prescribed exercises across a wide range of conditions. Whether someone attends our Drogheda physiotherapy clinic for back pain, neck pain, pelvic floor dysfunction, urinary urgency, constipation, bloating, poor posture, or reduced core stability, breathing retraining is often an important piece of the puzzle.

The reason may surprise you.


At first glance, breathing seems far too simple to have such a significant effect on health and recovery. After all, we do it automatically thousands of times every day. However, the way we breathe influences far more than just oxygen intake. Breathing patterns affect the nervous system, digestion, posture, core function, pelvic floor health, pain levels, stress resilience, and overall wellbeing.

This is why diaphragmatic breathing remains one of the most effective and widely prescribed tools used by our physiotherapists at BodyRight Clinic.


What Is Diaphragmatic Breathing?


Diaphragmatic breathing involves using the diaphragm—the primary muscle of respiration—to take slow, controlled breaths. As the diaphragm contracts, it moves downward, allowing the lungs to expand fully. The abdomen gently rises on the inhale and falls on the exhale.

This is how most of us naturally breathe when we are relaxed, calm, and feeling safe.

Many people unknowingly develop shallow chest breathing patterns due to stress, pain, poor posture, injury, prolonged sitting, or ongoing muscle tension. Over time, these habits can contribute to increased stress levels, reduced core support, digestive dysfunction, pelvic floor tension, and persistent pain.


How the Nervous System Influences Breathing


When we feel calm and relaxed, the body operates primarily through the parasympathetic nervous system—often called the "rest-and-digest" system. During this state, breathing naturally becomes slower, deeper, and more diaphragmatic.

This state is associated with:

  • Reduced stress and anxiety

  • Lower heart rate and blood pressure

  • Improved digestion

  • Reduced muscle tension

  • Better sleep quality

  • Enhanced recovery and healing

The interesting thing is that breathing doesn't just reflect the state of the nervous system—it can also influence it.


By deliberately slowing the breath and encouraging diaphragmatic movement, we can help guide the nervous system back towards a more relaxed, restorative state.

When we are stressed, however, breathing often shifts into the upper chest. We begin to use the accessory breathing muscles around the neck, shoulders, and upper ribs.

This is extremely useful if you're running up a hill, exercising hard, or escaping a genuine threat. It forms part of the body's "fight-or-flight" response.

During this state:

  • Digestion slows down

  • The bowel becomes less active

  • Heart rate increases

  • Muscles become more tense

  • Stress hormones such as adrenaline and cortisol rise

  • The body and brain become more vigilant and alert

These responses are designed to help us survive emergencies.


The problem is that modern life often keeps people in this state for much longer than intended. As a result, muscles can become chronically tight and painful, digestion can become disrupted, pelvic floor muscles can become overactive, and the body's natural pressure-management system—our core—can become less efficient.


This is where diaphragmatic breathing becomes such a valuable tool. Even a few minutes of focused breathing can help signal to the body that it is safe to relax. It can be a remarkably effective "reset button" for the nervous system.


Breathing for Improved Core Stability and Posture


The diaphragm is much more than a breathing muscle. It works closely with the deep abdominal muscles, pelvic floor, and spinal stabilisers to create a supportive "core canister."

When the diaphragm functions properly, it helps regulate pressure within the abdomen and provides stability for the spine and pelvis. Poor breathing patterns can disrupt this system, leading to increased strain on surrounding muscles and joints.

This is one reason diaphragmatic breathing is commonly prescribed for:

  • Back pain

  • Neck pain

  • Poor posture

  • Core weakness

  • Overactive pelvic floor dysfunction

  • Pelvic pain

By restoring proper breathing mechanics, we can often improve movement efficiency, posture, and reduce unnecessary muscular compensation.


Better Pain Management


Pain and breathing are closely linked. People experiencing pain frequently hold their breath or adopt shallow breathing patterns without realising it. Think back to the fight-or-flight response again.

This can increase muscle guarding, tension, and sensitivity within the nervous system.

Diaphragmatic breathing can help:

  • Reduce pain-related muscle tension

  • Improve relaxation during movement and exercise

  • Decrease nervous system sensitivity

  • Improve body awareness

For many patients with persistent back pain and neck pain, improving breathing mechanics is an important component of long-term recovery.


The Link Between Breathing and Digestion


The diaphragm sits directly above the stomach, liver, and intestines. With every breath, it gently moves up and down, creating a natural massage-like effect on the digestive organs. This movement helps support circulation, lymphatic flow, and the movement of food and gas through the gastrointestinal tract.


Diaphragmatic breathing also stimulates the vagus nerve, one of the body's most important communication pathways between the brain and the digestive system. Increased vagal activity encourages the body to shift into a state where digestion can function more efficiently.


When we are stressed, breathing often becomes shallow and chest-dominant. This activates the body's fight-or-flight response, which can slow digestion and contribute to symptoms such as:

  • Bloating

  • Abdominal discomfort

  • Indigestion

  • Constipation

  • Increased gut sensitivity


The Link Between Bloating and Breathing


Many people assume bloating is simply excess gas or fluid in the abdomen, but in some cases the body's muscular response plays a significant role.


Research has shown that some individuals with bloating develop altered coordination between the diaphragm and abdominal wall, a phenomenon known as abdominophrenic dyssynergia. Instead of relaxing and returning upward during exhalation, the diaphragm remains lower and more contracted while the abdominal wall relaxes and protrudes outward.

This can lead to:

  • Increased abdominal distension

  • Feelings of pressure and fullness

  • More shallow breathing

  • Reduced core support

  • Less efficient pressure management throughout the abdomen and pelvis

Diaphragmatic breathing can help restore more normal coordination between the diaphragm, abdominal muscles, and pelvic floor, often reducing the severity of symptoms.


The Power of a Longer Exhale


While diaphragmatic breathing is beneficial on its own, adding a slightly longer exhale can further enhance its effects.

One of my preferred ways to teach this is to gently slow the breath at the back of the throat during exhalation. Rather than simply blowing the air out, imagine creating a soft, quiet "haaaa" sound, as if you were lightly fogging up a mirror.


This gentle narrowing of the airway naturally lengthens the exhale and prevents the breath from rushing out too quickly. Think of it as lightly applying the brakes to the breath.

A longer exhale helps stimulate vagal activity and encourages a stronger parasympathetic, or "rest-and-digest," response.

  • Lower heart rate

  • Reduced muscle tension

  • Improved digestion

  • Better sleep quality

  • Enhanced recovery from injury and exercise


For many patients, the "haaaa" exhale feels more natural than counting breaths and is easier to incorporate into daily life.  Just a few expansive Belly Breaths with long sighing exhales are wonderful to practise through your day to help the body and nervous system in different ways.


Supporting Bowel Function, Constipation and Bowel Urge


One of the most rewarding benefits of a lengthened exhale is its effect on bowel function.

Many individuals with constipation spend much of their day in a sympathetic, stress-dominant state. In this environment, digestive processes are often slowed, and the body's signals that it is time to empty the bowels may become less noticeable.


By slowing the breath and lengthening the exhale, we encourage a shift toward the parasympathetic nervous system. This can support:

  • Improved bowel motility

  • Greater awareness of bowel urge

  • Reduced abdominal tension

  • Relaxation of the pelvic floor

  • Better coordination between the diaphragm and pelvic floor


Many patients notice that a few minutes of diaphragmatic breathing with a slow "haaaa" exhale—particularly in the morning or while sitting on the toilet—can help stimulate the urge to open their bowels without straining.

As we often tell patients, the bowels work best when the body feels safe enough to let go.


Breathing and Overactive Pelvic Floor Dysfunction


The diaphragm and pelvic floor function as a team. During inhalation, both structures gently descend. During exhalation, they naturally recoil and rise.

For individuals with an overactive pelvic floor, urinary urgency, pelvic pain, constipation, or difficulty emptying the bowels, excessive tension within the pelvic floor muscles can become a significant contributor to symptoms.


Learning to breathe diaphragmatically can help reduce unnecessary pelvic floor tension and improve coordination between the diaphragm, abdominal wall, and pelvic floor. This is one reason breathing exercises are often a foundational component of pelvic floor physiotherapy.

Breathing is something we do thousands of times every day without thinking about it. Because it is automatic, many people assume that breathing exercises should be simple and intuitive.


In reality, learning diaphragmatic breathing can be surprisingly challenging.

Years of stress, pain, injury, poor posture, pelvic floor dysfunction, digestive issues, or habitual chest breathing can make it difficult to coordinate the diaphragm, abdominal muscles, and pelvic floor effectively.


Sometimes letting go of tension can feel surprisingly difficult. Some people have spent years holding tension through their abdomen, chest, neck, or pelvic floor without even realising it. Others may have stiffness through the rib cage, spine, or abdominal wall that makes diaphragmatic breathing feel awkward or uncomfortable at first.


This is where skilled physiotherapy guidance can make all the difference.

When to practise your Diaphragmatic Breathing?


Weaving some Belly Breaths with long sighing exhales  into your day can be a wonderful ritual. Typically before we eat, as we settle to rest, when were on the loo, or sense tensions rising.  Also if you have an exercise routine which involves some stretching, that is a wonderful time to direct the expansive inhales to different areas, and feel the body release tension on the softening long exhales. 


Why We Prescribe It So Often


When you consider the diaphragm's influence on breathing, posture, core stability, digestion, pelvic floor function, bowel health, pain, stress, and recovery, it becomes clear why diaphragmatic breathing appears in so many treatment plans.


Whether the goal is reducing back pain, improving neck pain, calming an overactive pelvic floor, reducing urinary urgency, relieving constipation, managing bloating, improving posture, enhancing core stability, or simply helping the body recover more efficiently, diaphragmatic breathing provides a foundation upon which many other aspects of health can improve.
